AuthoraCare Collective is currently seeking a Nurse Practitioner for our Chronic Disease Management and Palliative Care. This position is Full-time Monday - Friday 8:00am - 5:00pm supporting our Burlington campus and service areas.

Job Summary:

Provide chronic disease management (CDM), palliative care (PC) and primary care services in outpatient settings, including the long-term care, home and/or office. The role of the CDM & PC Nurse Practitioner is to address the physical, psychological, emotional and spiritual needs of individuals with life-limiting or symptomatic issues. Provide direct patient care as needed. Participate in on-call coverage. Duties will also include teaching and education and assisting with program development, and community outreach. Assess and identify patient/caregiver issues, help guide and develop patient/caregiver care goals, and identify and implement interventions as appropriate for given setting.

Work with attending physician or primary care provider to establish and implement medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. May cover on-call for hospice and/or palliative care as assigned.
